Mats
Ines Ambite
Yes. Thank you, Catharina.
We are very happy to announce the publication yesterday of the Phase 2 trial results for the infection project that were published in Nature Microbiology, a very high impact journal. Infections are a major cause of disease and mortality, but antibiotics no longer work properly.
We need to create new options, and we have new options. The approach here is to target the disease, and not the bacteria.
What is actually the disease? During infection, the disease is caused when the pathogens, the bacteria in this case, infect the organs.
If they go to the lung, they create pneumonia, or diarrhea in the gut. For now, the treatment options are based on antibiotics, killing or inactivating directly the bacteria.
Those are effective against sensitive, but not against resistant bacteria that become a very big problem now. Our approach now is to treat the disease with inhibition therapy, to treat the cause of the symptoms and the disease, accelerate the bacterial clearance, and have action against resistant strains.
We now have four different molecules that target this disease, and not the bacteria, and that can create new non-antibiotic treatment therapy for infections. Four treatment approach: the IRF7, siRNA, MAP kinase, the lon, and the trial results are in with the IL-1 receptor antagonists.
Treatment effects: they inhibit the excessive innate immune response, they inhibit inflammation and the disease in the infected organs, and they also accelerate the bacterial clearance. Very importantly, they can also clear the bacteria even if those bacteria are antibiotic resistant, which is not the case with classic antibiotic treatments.
This Phase 2 trial was comparing the IL-1 receptor antagonist to a classic antibiotic treatment, and both therapies were able to treat the disease with similar efficacy. They were both treated the same way for 5 days, those patients, and followed up to 6 months after this initial treatment.
We have clinical data and a lot of molecular data to really follow the response to the treatment. Both treatments were effective long-term.
They reduced the symptom score in the patients, they reduced the recurrence rates, they improved the quality of life—that's very important because those are very difficult diseases that recur very often—they improved the bacterial flora, and also we saw rapid reduction of neutrophil degranulation, which is inflammation basically, and the cytokine storm response in the patients that were treated with the IL-1 receptor antagonist, but not in the patients treated with the antibiotics. We can conclude then that immunotherapy may be an alternative to antibiotics, and they can reduce the need for antibiotic treatments in this patient group.
They can reduce the selective pressure on antibiotic-resistant bacteria, and reduce the side effects on the normal flora in individuals and the population at large. I would encourage you to look at the whole study that was linked to in the press release yesterday.
Thank you.
Catharina Svanborg
This is really a great breakthrough in general for infectious disease treatment, for bacterial infection treatment, the fact that you can find the cause, as Ines said, and target the cause of the disease rather than just killing the bacteria. You even had some effects were even better than for the antibiotic.
We are extremely optimistic about this. I look forward to your questions as well after.
The other progress that we would like to report during this quarter is in the tuberculosis project, where Hamlet BioPharma has signed a collaboration agreement with a Korean company called ImmunoForge. The group of Gabriela Godaly are responsible for this project, but they are not here themselves today.
On February 3rd, we signed this agreement. The company ImmunoForge is a biotech company based in Seoul, Korea, and they develop new drug delivery technology, which is essential.
Tuberculosis is a slowly growing disease, very difficult to reach with substances, and you need to treat for a long time. Therefore, this slow-release delivery technology is something that we were very interested in.
It was ImmunoForge who contacted Hamlet BioPharma to initiate this collaboration because of the publications that have been in recent time. You can wonder why is it good to publish in Nature Microbiology.
It's because people find you. Hamlet Pharma of course holds all the rights to the peptide NCX and all of the work that has been developed by the Godaly group and by us.
The peptide has showed very good effects also against tuberculosis in animals. There has been some discussion evidently about this agreement, and it's important to say that the parties have agreed to share future IP rights and revenue arising from the collaboration, which means anything related to the technology of ImmunoForge, but Hamlet Pharma still retains all of the rights to the drug and any other use or any other collaboration with companies that could be interested in the commercialization of this product.
We see this as a very favorable agreement. It's also a company that has good standing in Korea, and you know that Korea has a very good biotech infrastructure, and tuberculosis is extremely common in Korea.
I have a little bit just an image—you can easily find this online—an image about the company and what they are doing. They say prolonged circulating half-life, this is to ensure long-term treatment of a single dose, and also they have mechanisms of delivering slow release, and you can follow the graphs here to see, compare just a single dose and how it disappears with their slow-release mechanism.
We find this very interesting. These are just some examples of the results on our side, where the Godaly group have shown that the NCX treatment makes mycobacterial growth much slower.
The effect is similar to rifampicin, which is the clinical drug of choice in tuberculosis today. These low blue parts here show that there is much less bacteria left than in the untreated sample.
These wonderful electron microscopy pictures show the gradual dissolution of the bacteria. You can see that they go from very sharp-edged molecules or things here, bacterial cells, and gradually after treatment they become dissolved, and of course they eventually die from this.
You can also see this here in the images where there are human cells near and talking to the bacteria. Very optimistic indeed for the future.
I will also end by commenting on the bladder cancer therapy, obviously. Actually, it happens to be Ines's profile here in the picture that you have seen before, and you also seen the drug in the bladder cancer tumor, and the dissolving tumor where the cells are shed by the patient in large numbers.
The Phase 2 study that has been completed had these three parts, I think you remember: a placebo-controlled first part with a low dose, then a dose-escalation part with two higher doses, and repeat treatment. This package of data has now been completed and written up.
The important endpoints here are safety, no serious adverse events, reduction in tumor size, complete or partial response, and a rapid acute response. You remember tumor cells shed within 2 hours, there is activation of an immune response, there is with pathology loss of tumor tissue and loss of cancer gene expression, a very strong effect.
What we are now planning, supported by the FDA process, is to do a neoadjuvant Phase 3 study, a so-called pivotal study. This is primarily a study where Alpha1H is given as a neoadjuvant prior to the TURBT, either in patients with primary tumor or in patients with recurrent tumors.
There are also other options for treatment of later stage disease, and we are exploring both of those currently, but the main focus and where we have been able to develop the process very far is for the neoadjuvant therapy. Just a summary since the second quarter had the communication with the FDA.
In November, Hamlet Pharma received pivotal study feedback. It was the FDA who suggested the pivotal study feedback, and which we think is very encouraging, and also helpful in advancing Hamlet's marketing objectives for Alpha1H.
The written feedback has really been guiding us in developing the new protocol and everything around the Phase 3 study. This study will show whether the treatment really works and is safe.
It's a different level of precision for this study to be granted marketing approval as a next step. We are finalizing the study protocol and plans, and this is not just talk.
There is this work to integrate the comments from the FDA into the protocol, to define the clinical sites, and to have all of the logistics around the study defined is a huge amount of work, and we are mastering that and we are very far ahead with this. As usual, we want to acknowledge our team in Lund in the lab, our wonderful legal and finance consultants, a lot of legal work currently, which is very interesting, key opinion leaders, the experts that's around us, and of course the clinical collaborators, and Linnane, the platform that are responsible for a lot of the practical work that is being done together with Hamlet Pharma.
Thank you very much. We look forward to your questions.
Mats
Catharina Svanborg
Ines, would you like to respond, or I can do it. It's okay.
Ines, you have lived with the statisticians and the legal comments and it's 24/7 for months and months.
I think very few people outside the field really understand how demanding these publications are.
Mats
Catharina Svanborg
Just det.
Catharina Svanborg
It's whether we are going to gradually partner.
Ines Ambite
Tack så mycket.
