Business
FOXO Technologies Inc. is a diversified healthcare services and technology company that provides epigenetic biomarker tools, clinical diagnostics, biospecimen sourcing, hospital operations, and behavioral healthcare services across the United States. Founded in 2020 and headquartered in West Palm Beach, Florida, the company operates through four primary business segments: Epigenetic Diagnostics and Interpretation, Rural Hospital Services, Mental and Behavioral Health, and Biospecimen and Clinical Sourcing. Through its subsidiary FOXO Labs, the company develops and commercializes artificial intelligence- and machine learning-driven DNA methylation profiling technologies; proprietary epigenetic algorithms that evaluate molecular health indicators, biological age, and mortality risk; non-invasive saliva-based collection kits and laboratory testing protocols; and predictive health analytics designed for life insurance underwriting, reinsurance risk stratification, and consumer longevity applications. Through Rennova Community Health, Inc. and its subsidiary Scott County Community Hospital, Inc., the company operates Big South Fork Medical Center in Oneida, Tennessee, providing acute inpatient care, 24-hour emergency room services, clinical laboratory diagnostics, radiology and diagnostic imaging, physical therapy, and outpatient clinical care to underserved rural populations. Through Myrtle Recovery Centers, Inc., the company operates residential addiction and psychiatric treatment facilities in East Tennessee, offering medically monitored inpatient detoxification; residential substance abuse rehabilitation; individualized psychotherapy and dual-diagnosis behavioral health care; and step-down outpatient recovery programs. Through Vector BioSource, Inc., the company supplies human biological specimens, whole blood, plasma fractions, and clinical data sets to pharmaceutical companies, biotechnology developers, and academic research institutions for drug discovery, clinical trials, and molecular diagnostic validation. In recent strategic developments, FOXO Technologies transitioned from a specialized epigenetic insurtech developer to an integrated multi-segment healthcare enterprise through a sequence of targeted acquisitions, including the consolidation of Rennova Community Health, Myrtle Recovery Centers, and Vector BioSource. In July 2026, Vector BioSource expanded its biological procurement network by entering into a definitive agreement to acquire four commercial blood and plasma collection centers across the United States from Grifols Bio Supplies, Inc. To facilitate corporate growth and capital management, the company established FOXO Acquisition Corporation to structure future asset integrations. Following its transition from the NYSE American to the over-the-counter market, the company executed corporate capital restructuring initiatives, including a 1-for-3,000 reverse stock split in June 2026, which initiated temporary trading under the ticker symbol FOXOD on OTC Markets prior to reverting to the core ticker symbol FOXO in support of long-term national exchange relisting objectives.
